Lost in Transition is a three-year ARC funded research project exploring the issues of young with complex support needs in transition that aims to map existing frameworks for transition support planning, describe the needs of young people with complex support needs in relation to key life transitions, develop an understanding of the various meanings of transition to young people with complex support needs and for those who provide transition support, and explore how these transitions can be tracked and quantified.
