CREATE Foundation will soon be providing all young people turning 16 and 17 with a Go Your Own Way (GYOW) kit as part of a new project to assist in the development of young people’s Leaving Care Plans and to support them to transition to independence successfully. The kit is filled with resources and information to assist the planning process and provide caseworkers, young people and carers with the information they need to plan for a successful transition.
